In return for your dedication, you'll receive:Salary of £25,500 per annumAccess to our Group Pension SchemeFlexible access to your earned wages to help manage unexpected expensesComprehensive health and wellbeing support, including:Mental health resources and counsellingLegal and financial adviceCareer coaching and life management supportHealth and wellbeing initiatives and challengesAccess to eLearning, career pathways and ongoing professional development through our award-winning Learning EnterpriseA supportive culture that values innovation, collaboration and continuous improvementModern mobile technology to support flexible community workingFree tea and coffee at your base locationAbout the RoleBased at Livingstone Community Hospital, Dartford, you'll work as part of our Community Nursing Team, supporting patients to receive high-quality care in their own homes and community clinics.This is a varied and rewarding role where you'll work alongside Community Nurses to provide compassionate, person-centred care that promotes independence, wellbeing and dignity.Our teams are equipped with modern technology, enabling you to access patient records and complete documentation remotely, helping you spend more time delivering care and less time travelling back to base.Located just off the M25, Livingstone Community Hospital benefits from excellent transport links and is within easy reach of Bluewater Shopping Centre and local amenities.Main ResponsibilitiesWorking closely with Community Nurses and the wider multidisciplinary team, you will:Deliver high-quality care to patients within their own homes and community clinic settingsSupport patients with a range of healthcare needs, including wound care, diabetes management and end-of-life careUndertake delegated clinical activities within your scope of competenceMonitor and report changes in patients' conditions to registered cliniciansPromote patient dignity, independence and wellbeing at all timesAccurately document care provided using electronic patient record systemsDevelop positive working relationships with patients, families and healthcare professionalsContribute to preventing unnecessary hospital admissions through effective community-based careSupport the continual improvement of services through shared learning and best practiceAbout YouWe're looking for a compassionate, motivated and organized individual who is passionate about delivering outstanding patient care.To be successful in this role, you'll have:An NVQ Level 3 in Health & Social Care, Care Certificate plus relevant experience, or equivalent healthcare qualification and experiencePrevious experience working in a healthcare, community or care settingStrong communication and interpersonal skillsExcellent organizational skills and the ability to manage a varied workloadConfidence using IT systems, including Microsoft Outlook, Word and ExcelThe ability to work independently and as part of a multidisciplinary teamEssential RequirementsFull UK driving licenseAccess to a vehicle for work purposesAbility to travel across the service area in a timely mannerWhy Join Us?This is an excellent opportunity to build your healthcare career within a supportive Community Nursing Team while making a real difference to people's lives every day.If you're passionate about delivering high-quality care and enjoy working in the community, we'd love to hear from you.
