Working alongside partners, community organisations, carers, and most importantly the people we support to shape meaningful and personalised outcomes. In your role as a social worker, you will work within the mental health social care team, supporting adults with mental health difficulties who require social care input. Your practice will be strengths-based and person-centred, working closely with health colleagues and partner agencies to promote recovery, reduce delays, support safe transitions and help people achieve the outcomes that matter to them.As part of the role, you will also hold additional responsibility as the team's AMHP. This will include dedicating one day each week to the Locality AMHP rota, undertaking Mental Health Act assessments and making safe, lawful decisions to support people in mental health crisis.
