Eden Brown synergy are currently recruiting for Bury Council who are looking for Hospital Social Worker Hospital Social Worker Bury Council £30.00 per hour | Locum Opportunity Bury Council is looking for an experienced Hospital Social Worker to join its busy Hospital Team. This is an excellent opportunity for a Social Worker with previous hospital discharge experience who thrives in a fast-paced multidisciplinary environment. Key Responsibilities Complete Care Act assessments and discharge planning. Support safe and timely hospital discharges. Work collaboratively with healthcare professionals, patients, and families. Manage a varied hospital caseload across multiple hospital sites. Promote independence and person-centred outcomes. Requirements Qualified Social Worker registered with Social Work England. Previous experience within a Hospital Discharge Team is preferred. Strong knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding. Ability to work across multiple hospital sites. Willingness to participate in weekend working.
